Definition of hip pain

Hip pain is a common symptom of various pathologies and depending on the patient's clinical condition it may be related to different injuries. Germán Norambuena, a traumatologist who specializes in hip at Clínica Dávila, says: “In general, hip pain has multiple locations, but the most frequent is at the groin level. It can also hurt from the lateral portion, which leads to tendon pathologies of the segment. It can also hurt in the gluteal region or even radiate to the knee. It is important to keep in mind that a large part of the patients that we see in consultation for a condition in the hip have symptoms that make us think of some pathology related to the lower lumbar spine ”.

Both those who practice a sport routinely, and those who are insufficiently active, are likely to develop some type of pathology in the hip. While structural overload is important when determining the exact diagnosis of each individual, other factors should be considered, such as: age, occupation, family and medical history. Some injuries are attributable to birth malformations or to pathologies that at some point in adolescence were not investigated or treated in a timely manner. At the hip level the most frequent pathologies are osteoarthritis and femoroacetabular impingement (also called “hip impingement”, and that usually occurs as a result of abnormal rubbing of the hip bones with each other). These alterations can greatly hinder the development of daily activities and generate a high impact on people's quality of life.

Associated with the excess of physical activity or the performance of sports with an inadequate technique, tendinopathies appear. Frequently the gluteus medius tendon is affected. In case of chronic conditions there may be inflammation in the tendon insertion site and generate enthesopathy; In this case, the bone is involved in the clinical picture.

Another frequent cause associated with hip pain is a certain degree of irritation on the bursa, (structure that is strategically located in some joints to reduce the friction generated by tendons with movement). Bursitis usually appears when body compensation is not enough to control the underlying problem.

In a smaller proportion of hip pathologies are the fractures of the pelvis and femur, which, although they generate local pain in the hip, are characterized by presenting a clinical picture of greater intensity with an important level of functional impotence: people suffer enough pain , which forces them to seek immediate medical assistance.

If we consider other segments adjacent to the hip, we must consider the anatomical complexity of the pubis. Because of an increase in traction generated by the abdominal musculature or the adductor musculature, local inflammation can be generated in said joint that is characterized by pain radiating to the hip.

Pulp nucleus hernias or facet syndromes are lumbar conditions that contribute to hip discomfort. The pain generated in the spine is usually perceived by the posterior region of the hip, and in those cases in which the sciatic nerve is irritated or inflamed, it can radiate to the foot through the back of the entire lower limb.
